ONE (1) GRAND PRIZE: A 2013 Ford Fiesta ST (ARV $26,000). Grand Prize Winner is responsible for vehicle registration, license, tax, title and insurance fees and for all costs associated with any available upgrade/option not provided with the prize vehicle and any other expenses related to the acceptance and use of the prize not specified.

WEEKLY PRIZES: Up to thirty-five (35) prizes will be awarded – approximately five (5) per Weekly Period. Prizes will consist of Octane Academy merchandise. Each prize will have an Approximate Retail Value (“ARV”) of $10 – $250.

A total of seven (7) Weekly Prize winners will be selected (one per Weekly Entry Period) in random drawings according to the prize drawing schedule above from among all Entries received for the applicable Entry Period. One (1) Grand Prize Winner will be selected in a random drawing conducted on or about 8/1/13 from among all eligible Entries received during the entire Sweepstakes Period. Drawings will be conducted by Administrator, an independent judging organization. Entries not selected for any Weekly drawing will be retained for subsequent drawings. Administrator will attempt to contact potential winners by phone. Potential winners must be able to receive calls from a blocked/unlisted number. If a Ford Octane Academy No Skills Required Sweepstakes potential winner timely responds, Administrator will orally request information from the potential winner to confirm preliminary eligibility. If a potential winner is deemed potentially eligible based on the preliminary questions, Administrator will send an Affidavit of Eligibility and Liability and, unless prohibited by law, a Publicity Release form (“Affidavit/Release”) to the potential winner by email or facsimile to be executed and returned by email or facsimile within seven (7) days from the delivery of Administrator’s email or facsimile. If the potential winner cannot be contacted within the specified time after the first attempt to contact him/her, if he/she fails to execute and return the Affidavit/Release within seven (7) days, if he/she is otherwise ineligible to receive the prize, that potential winner will be deemed ineligible. An alternate will be selected in his/her place and Administrator will attempt to contact and obtain the required Affidavit/Release from that potential winner. If any Weekly Prize remains un-awarded after three (3) attempts by Administrator, that Prize will not be awarded. Except where prohibited by law, acceptance of prize constitutes permission for Sponsor to use winner’s name, hometown, likeness, statements and other personally identifiable information for promotional, advertising and marketing purposes in any media throughout the world without additional prize, incentive, consideration, consent or review. If a Ford Octane Academy No Skills Required Sweepstakes winner is disqualified for any reason an alternate winner will be selected. Odds of winning any drawing depends on the number of eligible Entries received for that drawing. Limit: One (1) Weekly Prize per household. Weekly Prize winners are eligible for the Grand Prize drawing.

If you do not answer the question correctly on the first try, you will be allowed to try again until you do so successfully. Watching Long Island Medium on TLC will give you information that will help you answer the question. Note that there is a limit of one entry per person/email address per entry period.

Prize: a trip for the winner and one guest to Long Island, NY, for a private reading from Theresa Caputo. The prize consists of round trip coach air travel for two people to New York, NY, from a major commercial airport near winner’s residence (as determined by Sponsor); one doubleoccupancy hotel room for 3 days/2 nights; a private reading with Theresa Caputo; ground transportation; and $500 USD spending money. Meet the Medium Sweepstakes Potential winner will be notified by email, phone, postal mail and/or express mail at Sponsor’s
sole discretion. Inability to contact a potential winner or the return of any prize notification as undeliverable will result in the prize being forfeited and an alternate winner will be selected at
random from the remaining eligible entries. Return of any prize notification or prize as undeliverable may result in forfeiture of the prize, in Sponsor’s sole discretion. Potential winner
will be required to complete an affidavit of eligibility, liability and (where legal) publicity release (or in Canada a Declaration of Compliance) which must be returned within time period specified

By accepting the Grand Prize, the Meet the Medium Sweepstakes winner and his/her guest represent that they understand that travel, whether by plane, automobile, or other conveyance, or by foot, contains some inherent element of risk of accident, illness, injury, loss or death, which may be caused by negligence, forces of nature, or other agencies, known or unknown. Meet the Medium Sweepstakes Grand Prize winner and his/her guest represent that they recognize that such risks may be present at any time before, during and after the trip. In order to be considered the Grand Prize winner and partake of the enjoyment of this trip, Grand Prize winner and his/her guest must be willing to accept the risk, and to confirm their acceptance to such risk in writing at Sponsor’s request.

The approximate retail value of the Weekly Prize is $500, subject to OTC Gift Card terms and conditions. Winners will be drawn within one (1) week after the close of the Weekly Entry Period. One Morris 4X4 Center Jeep Giveaway Grand Prize Winner will be selected in a random drawing on or about 04/05/2014 from among all entries properly received. Drawing will be conducted by Garvin Promotion Group, LLC, an independent judging organization whose decisions are final.
