Cost Plus World Market Fathers Day Sweepstakes Potential winners will be notified by phone, email and/or mail and potential winners will be required to execute and return an affidavit of eligibility/liability release, tax form and, where legal, a publicity release (collectively, the “Eligibility Documents”) within five (5) calendar days of date of issuance. If Cost Plus World Market Fathers Day Sweepstakes potential winners cannot be reached within five (5) calendar days from the first notification attempt, or in the event of noncompliance within the above stated Eligibility Documents time period, with these Official Rules, or if prize notification is returned as undeliverable, the prize will be forfeited and, at Sponsor’s sole discretion, an alternate winner may be selected. Administrator is an independent judging organization, whose decisions as to the administration and operation of the Sweepstakes and the selection of the potential winner are final and binding in all matters related to the Sweepstakes. Administrator will randomly select the NHL 14 EA Cover Vote Sweepstakes potential Sweepstakes winner from all eligible entries received during the Promotion Period, on or around May 27, 2013 in Pleasant Ridge, MI, U.S.A. The NHL 14 EA Cover Vote Sweepstakes potential winner will be notified by email, mail or phone, at Sponsor’s or Administrator’s discretion. The potential winner will be required to sign and return a release form (where permitted by law) affirming his or her eligibility to participate, releasing the Released Parties (as defined below) from liability and consenting to the Released Parties use of the winner’s name and likeness for promotional purposes (where permitted by law) (the “Release”), which must be received by Administrator within two (2) days of the date notice or attempted notice is sent, in order to claim the prize. If the potential winner is a Canadian resident he/she will be required to correctly answer a mathematical skill-testing question without assistance of any kind, whether mechanical, electronic or otherwise, in order to be eligible to receive the prize. Except where prohibited, the potential Grand Prize winner (parent/legal guardian if a winner is a minor in his/her place of residence) will be required to sign and return a Declaration of Compliance, Liability and Publicity Release
(“Declaration”), which must be received by Administrator within seven (7) days of the date notice or attempted notice is sent, in order to claim his/her prize. If any potential winner is a Canadian resident he/she will be required to answer a time limited skill testing question without assistance, in order to be eligible to receive a prize. Receiving a prize is contingent upon compliance with these Official Rules. If a potential winner cannot be contacted, fails to
sign and return the Declaration within the required time period (if applicable), fails to answer the skill-testing question correctly (if applicable), or prize is returned as undeliverable, potential winner forfeits the prize. In the event that a Rosetta Stone Embrace the Adventure Sweepstakes potential winner is disqualified for any reason, Sponsor will award the applicable prize to an alternate winner by random drawing from among all remaining eligible entries. Only three (3) alternate drawings will be held, after which the applicable prize will remain un-awarded. Prizes will be fulfilled approximately 8-10 weeks after the conclusion of the Sweepstakes.

Regardless of method of entry, each entrant may enter two (2) times during the Promotion Period.
