Verizon Wireless The Perfect Day Giveaway Winners will be contacted via phone and/or email and will be required to provide his/her full name, address (no P.O. Boxes), city, state, zip code and date of birth via email within seven (7) days of notification. Noncompliance within this time period will result in disqualification and an alternate winner will be selected. If any Verizon Wireless The Perfect Day Giveaway winner fails to comply with the above timeframe, if any prize or prize notification is returned as non-deliverable, or if any winner rejects his/her prize or in the event of noncompliance with these Official Rules, such prize will be forfeited and, an alternate winner will be selected. Upon prize forfeiture, no compensation will be awarded. If winner is a minor in his/her state of residence, prize will be awarded in the name of his/her parent or legal guardian. Decisions of judges are final with respect to all matters relating to the Sweepstakes. Prize award is subject to verification of eligibility and compliance with these Official Rules.
